The Form 759-C is completed by localities and sent monthly to TAX to report local assessments of tax. Localities that use an automated system for tracking assessments may instead submit a facsimile Form 759-C produced by their system. The facsimile must provide the same information as the Form 759-C. Detailed instructions are also provided to complete the Form 759-C. (Effective January 2009)

Two versions of the form are provided.
PRINTABLE
Print blank copies of the "Form 759-C Printable" version if you wish to have paper copies that are filled in by hand.
FILLABLE
Use the "Form 759-C Fillable" version if you wish to complete the form on your PC. The fillable document may be saved on your PC and printed if you wish a paper copy.
