Local Treasurers fax this form to TAX annually after submitting their Local Uncollectible List to TAX via EESMC or on-line in IRMS. This Transmittal Form reflects changes made based on the redesign of Form 759, which also affects the way the Uncollectible Balance is calculated. The Uncollectible Advice Amount statement reflects the change in how the Advice Amount is now calculated. (Effective January 2010)
