Make copies of the form on PINK paper.
1. Cut the two forms apart and use one half sheet for each bundle of denied accelerated refund returns.
NOTE: The on-line keyer should enter the information in the appropriate fields of the LOCAL DENIED Document. The completed document is later attached to the bundle of returns before they are shipped to Virginia Tax.
